Dear Parents/Guardians of students entering Kindergarten, 7th and 12th grades, and any new students to Union School District.

As a reminder, the Pennsylvania Department of Health requires that students entering Kindergarten receive the State required immunizations for entry into school.  This information was in your child's registration packet that was received in the spring.  Proof of immunizations must be received by the 1st day of school, which is August 21, 2024.

For students at Union High School, letters were sent home in May - notifying parents/guardians of incoming 7th and 12th graders, who have not previously submitted verification of the required immunizations.  If you are unsure if your child is up to date on their vaccines, please contact their PCP for verification prior to the start of the upcoming school year.

Students entering 7th grade, must receive the TDAP vaccine and the first Meningitis Vaccine (MCV).

Students entering 12th grade are required to have the second MCV vaccine.

Per Pennsylvania School Health Law, verification of immunizations must be received for entry in these grades, or students risk exclusion from attending classes and any extracurricular activities. Documentation of immunizations MUST be received by August 21, 2024.

Immunizations are available at your child’s PCP office, Primary Health Care in Clarion, The Clarion County Health Department, and most Walmart pharmacies.

Please submit record of vaccines to the nurse in your child's building.

Sincerely,
Mrs. Mary Sweeney, BSN, RN, CSN
Union School District Nurse
